Wellbeing Coordinator – Linking Leeds
NJC Scale Point 12 (£28,598 pro rata)
17.5 hours per week
As a Wellbeing Coordinator, you’ll play a key role in Linking Leeds. You’ll support individuals to access non-medical support by completing holistic assessments, co-creating personal Wellbeing Plans, and linking people to community groups and services that boost mental, physical, and social wellbeing. Working across GP practices, community venues, and occasionally from home, you'll help people build resilience, independence, and a better quality of life.
We are looking for someone with a passion for reducing health inequalities and promoting diversity, excellent interpersonal and communication skills, with the ability to build trusting relationships and someone who’s comfortable working across different locations.
This is an exciting opportunity to join Leeds Irish Health & Homes (LIHH) as part of the Linking Leeds partnership. This role covers the Wetherby area, supporting local residents while working with GP practices, community venues, and occasionally from home.
Linking Leeds is a citywide social prescribing service connecting people to community services and activities to improve health and wellbeing. It is funded by the NHS West Yorkshire Integrated Care Board and led by Inspire North, in partnership with BARCA, Leeds Mind, Feel Good Factor, Better Leeds Communities, Leeds Irish Health and Homes, Age UK and Leeds City Council.
We require you to:
· Manage a caseload, assessing needs and developing support plans
· Support people in Wetherby to access local services and activities
· Build relationships with GP practices and community partners
· Empower individuals to improve wellbeing and independence
· Maintain accurate records and support monitoring requirements
We need you to have:
· Experience supporting vulnerable people
· A commitment to reducing health inequalities and promoting diversity
· Strong communication and interpersonal skills
· Ability to build trusting relationships
· Good organisational skills and flexibility
